address and citizenship of applicant (if an individual) or jurisdiction (if a juridical person);
- Certificate of good standing or certificate of incorporation, issued by the Secretary of the State, Companies Registrar,
Administration of Industries and Commerce, Chamber of Commerce or equivalent entity (if applicant is a juridical person) (*)
